Purpose: The Columbia County Senior Center’s Home Delivered Meals (HDM) program provides support to eligible seniors experiencing challenges that prevent them from preparing meals or accessing the Senior Center in person.
Eligibility Requirements

Additional qualifying conditions include:
1. Inability to Prepare Meals: The individual is physically, emotionally, or mentally unable to prepare their own meals.
2. Inaccessibility of Senior Center: The individual cannot attend the Senior Center due to physical, emotional, or mental limitations, or lacks transportation—even if provided by the Center.
Ongoing Assessment
For participants requiring long-term service, a bi-yearly assessment will be conducted to verify continued eligibility and need
Intake and Assessment Process
When a request for Home Delivered Meals is received, the following steps are followed:
1. Phone Screening-The Home Delivery Coordinator conducts an initial phone interview to determine if the applicant meets the basic eligibility criteria.
2. In-Home Assessment- If the applicant qualifies during the phone screening, the Home Delivery Coordinator will arrange an inhome visit to go over the Home Delivery Meal Program.
3. Program Enrollment- The applicant will then be enrolled in the program.
